Phoenician Hospice Program


The Phoenician Hospice Team is dedicated to providing innovative end-of-life care with dignity and respect. This team is here to meet your needs 24 hours out of every day by providing:  

  • Regular visits by registered hospice nurses with specialized training and expertise in pain and symptom management. 
  • Visits by certified palliative hospice aides to provide personal care and attention. 
  • Live access after-hours to the Phoenician Hospice Team including a Triage RN. 
  • Psychosocial support for patients and families. 
  • Spiritual support for patients and families, including support groups. 
  • Trained volunteers who provide companionship, assistance, and support. 
  • An in-depth intake/referral process. 
  • Unique rapid-response programs aimed at avoiding re-hospitalization. 
  • Enhanced communication with families, facilities & group homes.
  • Access to valley wide mobile medical providers including MDs, NPs, & PAs. 
  • Direct, timely access to an RN for clinical concerns. 
  • Bereavement counseling and support for family members and significant others throughout the patient's illness and for a minimum of 13 months. 
  • Access to a Grief Recovery Specialist. 
  • MD & NP availability for symptom management visits.
  • Coordination of medical equipment and medications related to the terminal illness for comfort and symptom management. 
  • Physical, occupational, speech, music, and other therapies as indicated in the Plan of Care.

Our Programs


The Bereavement Program

Our Sacred Journey support group strives to provide support and education by helping loved ones build strength, resilience and hope for the future. 

  • This includes calls/visits.
  • Grief literature.  
  • Annual memorial services. 
  • Bereavement mailings. 

The Volunteer Program

 “Volunteers do not necessarily have the time; they just have the heart.” ~Elizabeth Andrew  

  • Emotional and practical support 
  • Respite to caregivers. 
  • Engaging in patient hobbies, current events and conversation.

Phoenician Outreach Program

Patients in the Outreach Program can continue to seek aggressive treatment or you may have graduated from hospice. The Outreach Program Social Worker will assist you with coordination of care and support. 

  • Outreach program is provided at no cost to patient or family. 
  • Social Worker will work closely with primary care physicians.
  • Access to Livi Home Medical, Home Health Care, Durable Medical Equipment, private caregiver options and much more.
  • Advanced care planning/directives. 
  • Enhanced communication with patient and families at a minimum of once a month via phone or email.

Vent Release

Our vent release process provides a rational approach to ventilator care and eventual liberation from the ventilator in a safe, efficient and ethical manner. This protocol will involve the Phoenician Hospice interdisciplinary team comprising of  the SW, RN, HHA, and Physician, including the Respiratory Therapist. Our goal is to provide patient comfort and dignity throughout the palliative ventilator release process.

Music & Massage Therapy

Music therapy has shown promise in both motor control and emotional functions. Also it has been around for thousands of years because of its influence on peoples emotions. Our Music Therapy Program targets to improve the quality of our patients lives.



Our Massage Therapy Program consists of licensed massage therapists who emphasize geriatric massage techniques which are directed toward the amelioration of pain.
