What is Outreach?


When someone you care for is faced with issues around aging, serious illness, or death, many of us turn to faith, spiritual leaders, and health care professionals for support and guidance. This is where the Phoenician Outreach Team can help! We offer information, education, support, and spiritual guidance to you and your loved one to navigate through these important topics and decisions.

Why Phoenician Outreach?


Both outreach and hospice care provide comfort. The Phoenician Outreach Program works to ease the physical and emotional suffering of the patient and improve the quality of life for both the patient and their family by providing a higher level of support and communication. A patient is able to to qualify for the outreach program anytime during their disease process. Outreach services are available in the time preceding hospice, where the patient may not yet be appropriate or be ready for hospice. The Phoenician Outreach Program is a FREE SERVICE coordinated by Phoenician Hospice. 

Phoenician Outreach Program Details


The Phoenician Outreach Program includes but is not limited to:

  • Education regarding the disease process and prognosis facing the patient.
  • Advanced care planning and decision making support-help families understand their options and provide guidance when making important care decisions for your loved one. 
  • Coordination of care and exceptional communication between patient, family, and care team. 
  • Full access if to Mobile Physician Providers if patient is needing more care.
  • Access to resources for medical equipment. 
  • Access to emotional and spiritual guidance.
